Our Admissions Process
- Initial Conversation — Contact us at (202) 438-9779 to discuss your loved one’s needs, preferences, and current situation.
- Private Tour — Visit the home, meet our team, and ask questions.
- Resident Assessment — We review the prospective resident’s health, mobility, daily-care needs, medications, and personal preferences.
- Admission Review — Our team determines whether HALOM Assisted Living can safely provide the appropriate level of care.
- Agreement and Move-In Planning — Once accepted, we complete the required forms, Resident Agreement, and personalized move-in plan.
Information Needed for Admission
- Completed admission application and resident information
- Recent physical examination and healthcare practitioner’s assessment
- Current medication list, prescriptions, and medication orders
- Medical history, diagnoses, allergies, and current care needs
- Photo identification and insurance information
- Emergency contacts and authorized representatives
- Advance directive, MOLST, power of attorney, or guardianship documents, when applicable
- Any additional records requested during the resident assessment
